Negotiations between the two clubs over the German star have been ongoing several weeks but took a significant step forward over the past 48 hours with officials verbally agreeing a £90million total fee.
The west London club will pay a guaranteed £71.8m to Bundesliga side Leverkusen, with the rest made up of performance-related bonuses.
Formally agreeing personal terms is unlikely to bean issue, with Kai Havertz set to earn a minimum of £130,000-a-week on a five-year-deal. Those wages will increase if he meets bonus targets.

Chelsea are on the brink of snap up another marquee signing after reportedly agreeing a £50million deal with Leicester City for left-back Ben Chilwell.
The Blues have made a statement of intent this summer, signing Hakim Ziyech for £38m, Timo Werner for £53m, while they are also agreeing deals for Thiago Silva, Kai Havertz and Malang Sarr.
And according to The Telegraph, claims the Blues are now expected to complete the signing of England international Ben Chilwell within the next three days after agreeing a £50m fee with Leicester.
The 23-year-old left-back will sign a five-year contract at Stamford Bridge as Frank Lampard’s side get ready to make inroads on England’s elite – Liverpool and Manchester City.
Chelsea set to sign former Nice defender Malang Sarr on four-year deal
Chelsea are poised to sign highly-rated defender Malang Sarr on a four-year deal, according to reports in France.
According to The Telefoot, claim the 21-year-old centre-back, who is a France youth international, will undergo a medical in London this week.
Sarr made 20 appearances for Nice during the 2019-20 season, but became a free agent in June after turning down a new contract with the Ligue 1 side.
Thiago Silva is expected to join Chelsea on a one-year deal
Brazilian defender Thiago Silva is expected to join Chelsea on a one-year deal in a surprise move from Champions League finalists Paris Saint-Germain.
Silva’s contract expired following Sunday night’s gut-wrenching defeat by Bayern Munich, meaning the 35-year-old Brazilian is available on a free transfer.
Blues boss Frank Lampard is determined to improve his defensive options this summer after the club conceded 79 goals in all competitions last season, and Thiago Silva could be one of multiple new recruits to arrive at Stamford Bridge.
The veteran will turn 36 next month, but Frank Lampard believes a player of Silva’s experience will significantly improve the back-line.
Thiago Silva won seven Ligue 1 titles over eight seasons at PSG, and he bid the club’s supporters farewell after the loss in Lisbon on Sunday night.
